Transcript Requests

The Family Educational Right and Privacy Act (FERPA) of 1974, as amended, requires the College to obtain an authorization from the student concerned for the release of a transcript.  Students may either request a transcript electronically through the secure Banner-Web site, or make the request by completing the appropriate form at the Registrar's Office at the Macon Campus, or the Warner Robins Campus in Warner Robins, or the Robins Resident Center on Robins Air Force Base.  The Transcript Request Form may also be printed by clicking on the form request link below and mailed to: The Office of the Registrar, Macon State College, 100 College Station Drive, Macon, GA. 31206-5144, or you may FAX it to (478) 757-2621. The turn around time from receipt of a request is generally within 3-5 days, except during registration, and there is no charge. Transcripts requested electronically through Banner-Web are printed and mailed the following business day after submitting the request.

IMPORTANT: Transcripts requiring grades for a term a student is enrolled cannot be printed until after the end-of-term process for that term is complete. For Spring 2008, grades are due by noon, May 6, with end of term processing projected to be completed by close of business May 8, 2008. Student transcript requests requiring Spring 2008 grades will be processed after that date.  Students requiring emergency service for a transcript with Spring 2008 grades before May 8, 2008 must contact the Registrar's Office as soon as possible.

Transcript Request Form  The form is a fillable Acrobat Reader form. The signature field will allow a digital signature if you have an up-to-date version of Acrobat Reader.  For more information on digital signatures, check your Acrobat Reader Help. If you do not have a digital signature, just skip this block.  After you have completed the information requested, just print then sign. You may mail the completed form, or FAX it to (478) 757-2621.
