Forms Explained

• Form A: Academic Renewal - Applies for students who have attended Macon State College previously. See page 2 of the form for eligibility.
• Form B: Application for Graduation - For all degrees/certificates (Fill-in PDF, print, and sign).
• Form E: Authorization to Disclose Grades - Used to send a term grade report to parents or employers. It is NOT a TRANSCRIPT request for admission to another institution. For a transcript request see main page.
• Form F: Evaluation of Credit from Non-Traditional Sources - The College does not award life experience credit
• Form G: Certificate of Immunization - Current through Fall 2007
• Form H: Certificate of Immunization - Spring 2008 - Effective for new students Spring 2008. Students should note requirements on page 2 of form
• Form I: Change of Major - 2007 - 2008 Catalog
• Form J: Change of Name, Address, Phone # - Name changes require documentation such as a marriage license or court order submitted with request
• Form K: Former Student Admission Application - No fee charged. (Fill-in PDF and email to registrar)
• Form M: Board of Regents Residency Appeal and Instructions - Appeal of Residency Determination
• Form N: Student Petition Form - Academic
• Form O: Transient Permission Request - (Fill-in PDF, print, and sign) Follow instructions on form for processing. Required for Macon State College students attending other institutions and requires a student's advising division review and approval.
