Group life insurance is provided to employees who consistently work 30 hours or more per week. Employees receive $25,000 of life insurance at no cost. This coverage includes double indemnity for accidental death and coverage for dismemberment due to an accident.
Supplemental life insurance is available in amounts equal to one, two, three, four, or five time(s) the employee's annual salary. This insurance is optional, and the employee pays the cost. The premiums are based on the age of the insured and the amount of coverage selected. Coverage up to three times the employee's salary does not require documentation. Coverage in the amount of four and five times the salary requires completion of an Evidence of Insurability (EOI) form.
Spouse Life Insurance is also available. The premium is based on age and the amount of coverage requested. Coverage may range from $10,000.00 up to $250,000.00
Child Life Insurance may be purchased for unmarried, dependent children up to age 19 (age 25 if a full-time student). Dependent coverage is $2000 for children to six months of age and $10,000 for all other eligible dependents. The cost for this coverage is $1.00 per month. This $1.00 premium pays for all covered children..
